[Lpk-execomm] Form for requesting event support

I had someone approach me about setting up events this year, and I 
suggested that to keep things sane that we should consider having a form 
to do this.  I went ahead and mocked up a form, and have placed it on 
our Drive.  It is available here:

I would like to ask that people seeking permission from the state party 
to do outreach events use this form, so we can standardize.

Additionally, if any of you believe I've missed something on the form, 
need to change something, or whatever; please let me know.  I am happy 
to modify the form accordingly.  I only built this to try to facilitate 
us, so that all of the information is outlined for consideration when 
there is a motion to allocate funds or whatever.

As a reminder, the use of the term "Libertarian Party" is trademarked by 
LP National, granted use to state affiliates, and those state affiliates 
can allow the use of the name to their affiliates.  So any event in a 
county where we do not have a county party today will require approval 
from the State Executive Committee for even the use of the name.
