[LPIL - Board of Directors] Survey 2023

I understand there are several platforms which can help us send/gather 
survey questions for free, and SurveyMonkey was one of them mentioned 
Tuesday.

Let me know how the IT Division can help with data collection / 
analysis; agree this is a worthwhile initiative.
